Dear Freedom of Information and Privacy Coordinator,
I am requesting the records held by Government related to Medical Assistance In Dying cases Canada, under the Freedom of Information Act. I am specifically requesting the records from past three years, from January1st, 2021 to January31st, 2023. 
I am precisely looking for the records of practitioners and hospitals (including their name and the method they used for MAID). Moreover, patients name and their medical history is much needed.
I have enclosed the  application fee in the form of cheque.
If there are problems releasing in any document, please don't put all the documents on hold but instead forward those you can share.

Reason:
Recently I came to know about this law of Medical Assistance In Dying, and I got to know the cases are rising day-by-day. So, I was curious to know the reason like why people choose this method to die and how practitioners or doctors performs it. In addition, this method still lacking the safeguards, data collection and oversight necessary to protect Canadians against premature death however according to some Canadians this is the easiest way to die (painless and quick). Therefore, i requested for the records of cases of patients to overview their medical history and the data for hospitals and doctors to know about the process of MAID.

  3. It is the first time I am learning about this topic or ever reading about it for the very first time. I am still trying to figure out that how this must work for the patients and how would the operators or the doctors handle these patients and is it really helpful?
